Skin Spots and Treatment

Skin spots showing a symmetrical distribution on the face, which are not congenital, are called maleas. Melasma is one of the skin problems that are difficult to treat. It is a problem mostly seen in young women, it is encountered in 10% of men. Treatment is difficult for people with dark skin.

What are the causes of skin spots?

Sunspots are divided into three as deep, superficial and mixed (both deep and superficial). The depth of the stain can be determined by wood light. Deep spots are difficult to treat.

What are the skin changes in melasma?

Melanization increases in melasma spots. The function and size of the color cells of the skin increases, the color cells move to the epidermis. The enzyme level that affects the color formation mechanism also increases. Color cells migrate to the deeper layer of the skin as the membrane protecting the dermis is damaged by the sun. Dermal blood vessels are activated by sun damage. Melanocytes are thus further triggered, and the maleae becomes deeper and more persistent.

What are the causes of melasma?

Sun exposure and genetic predisposition are the most important causes of these spots. Pregnancy triggers these spots. Increased estrogen and low progesterone are the reasons for this. Estrogen stimulates the receptors on the melanocytes and causes melanin secretion. Contraceptive has a similar effect. Ovarian dysfunctions and thyroid diseases can also cause staining.

Why Thulium Laser?

Various lasers of different wavelengths are used in spot treatment. High-energy ablative lasers stimulate carbon dioxide lasers and the stain destroys the color cells both on the surface and in the deep layer of the skin. In the carbon dioxide laser, the target is chromophore, ie water. The aim is to stimulate the epidermis and papillary dermis. The carbon dioxide laser coagulates the dermal vessels; In other words, new tightening is provided in dermal vessels. There is arisk of edema, swelling, pain and infection. Because epidermis and dermis are damaged. This can cause the person to stay away from daily life for a while. As the epidermis is affected, the risk of postinflammatory hyperpigmentation is increased.

In order to reduce the difficulties of carbon dioxide laser, erbium lasers, which make more superficial ablation, are also used in blemishes and skin rejuvenation. Erbium laser has more water absorption than CO2laser. The epidermis is rich in water, the stained superficial layer that is the epidermis; this laser is completely removed, but it does not provide coagulation in dermal vessels, it is not effective on dermal pigments. So erbium Laser is not effective on deep spots. Non-ablative fractional lasers only affect the dermis without damaging the epidermis. Fractional lasers with a wavelength of 1440-1550 nanometers create stimulation columns on the applied skin. The intact skin areas in between accelerate the healing, reduce the damage and do not detach the person from daily life. However, it has little effect on the stained part in the epidermis.

The 1927 nm wavelength Thulium Fractional Laser holds more water than the erbium laser, which is very effective on epidermis spots. Thanks to the wavelength, the effect goes deeper, and it destroys the melanocytes in the dermoepidermal junction. In other words, Thulium Laser has taken the comfortable parts of fractional and ablative lasers, it can do all the positive treatments that the two can do, providing faster recovery.
